This widget allows you to place Survey Monkey surveys on your web site. (Note that Survey Monkey is free but doesn't have a default thank you page.. so in order for this feature to work optimally you may need to upgrade so that your embedded survey doesn't redirect to a survey monkey page).
To insert this widget, use the following code:

Parameters
- sm - key parameter in emailing/embedding URL
- width and height define form dimensions, 300x400 is default, but you might want to copy it from embed code since size depends on fields you have
